Procedures
- The applicant shall submit the old cards and the enclosed documents to the Reception Desk officer.
- The form and the enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant pays the prescribed fees at the cashier’s counter.
- Submit the form and the enclosed documents to the Reception Desk officer.
- The Applications Intake officer enters data.
- The auditor receives the application, validates information against it, and checks its compliance with the health insurance system.
- In case of violations, the application is sent back to the Applications Intake officer to double check the entered dat
- In case the application is complete, the Audit and Posting officer will post the application.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant.
Required Documents
- The letter of retirement.
- The last salary statement from the Civil Service Retirement Directorate or Social Security Corporation.
- A cash payment for the next subscription period for the pensioner.
- A liabilities certificate, from Social Security Corporation, to deduct (3%) for the health insurance allowance.
- A copy of the Family Book.
- One (1) personal photograph.
